FastEthernet card not working in 12008 router

I have Cisco router 12008. Here is the "sh ver" output:

Also there is in SLOT 5 8 Port Fast Ethernet Copper card. The problem is, this card is not working. It tries to power up but after IOS download (I mean all that startup procedure these cards do in order to work) it puts itself into WAIT. Here is the "sh diag 5" output:

How can I make this card to work? Cards in slots 1, 2, and 3 work fine. Here is "sh diags summary" output:
