What type of fiber connector is on the NP-1F-D-MM FDDI module ? I have not seen this type before, it is not an SC,ST,LC.

Umm, FDDI. :)
I haven't seen any "official" name for it, but doing a search on "FDDI cable" should get you exactly what you need.
Ie. this place is way overpriced, but has what you are looking for.
