ONS 15454 question

The ONS15454 supports STS-12c-2v/VC-4-4c-2v concatenation when using the ML1000-2 card. VCAT spec only describes groups formed using STS-1 or STS-3c. What is the multiplexing rule for combining two STS-12c into one VCAT group? What is the byte ordering in the combined stream? Does the output pick up one byte from each STS-12c and mux them or does it pick up 4 bytes from each STS-12c at a time. Picking up 4 bytes would be similar to the order an STS-24c would have.
