READILY available as a _hardware-based_ attachment. Circa $1k per phone. "compatible" with similar units for landlines.
Skype uses AES-256 encryption, and it is available on some smartphones, but I am not sure if the smartphones support the encryption or not.
There are some flaws with it too:
