Do you have everything on 2-part forms or do you have a signature sheet only and give the customer a copy of the terms and conditions?

Do you have everything on 2-part forms or do you have a signature sheet only and give the customer a copy of the terms and conditions?
Two part forms. Customer initials and signs BOTH copies.
