I've had a ToggleLinc (pre-Insteon) switch in one bathroom to control a couple of ancient fluorescents for about a year. It's been flawless.
Today, it started turning off as soon as it turned on. It didn't matter whether I operated the switch or sent PLC commands.
Turning if off at the switch "fixed" it.
