LM14A problem

I have an LM14A, reliable for several years, that has started acting up. I wonder if anyone else has seen this behaviour. When it is turned off, it slowly fades as it always did but now, just before turning off, it flashes full on briefly before going off. Light bulbs normally last me for years but I had to replace this one after only brief use shortly after this odd problem appeared.

I had a similar problem: If I remember correctly, at first, I would find the lamp attached to the LM14A on at odd times. Then, when I'd try to turn it off, the lamp would dim, then flash back to full brightness and usually remain on. Strange.
