Doesn't surprise me much. There are over 3000 county-equivalents* in the US, so this means that only 3 percent of them are without 911 service. But most of the 109 counties in question are bound to be very extremely sparsely populated, so I would hazard a guess that their overall share of the US population is closer to 0.03 percent than it is to 3 percent.
Most states have counties, but Lousiana calls them "parishes" and Alaska has "boroughs". Other county-equivalents include the District of Columbia, and Virginia's "independent cities".
