Pacific Telephone in Southern California did.

113 - Information (later called "Directory Assistance") 114 - Repair service 116 - Emergency 117 - Test board (not advertised to the public)

This list reminds me of the codes we were taught to use on toll test boards at New England Telephone:


121 - Inward Operator
131 - Rate & Route
141 - Inforation
161 - Toll Test Board

I don't know if the "Toll Test Board" codes were used anywhere but AT&T: all the N.E.T. offices I worked with had regular phone numbers. I don't know why.
