In my opinion, the present arrangements are broken and indefensible.

There would be no need for a carrier or ISP to look into your affairs. You have an assigned telephone number and directory name, and that is to be transmitted. If you don't want that stuff transmitted to those you call, you may block such transmission at no charge. Blocking may be applied on a per call basis or on a all-call basis.
