I've inherited a leviton 49660 wire cutter/stripper. I've figured out
how to use the notches at the front to strip the outer jacket from cat
5 cable, and there's a cutter between the handles that seems self
explanatory, but I can't figure out what the other blade between the
handles is for. Since it doesn't meet the other handle, I'd guess it
is some sort of stripper for the paired conductors, but I don't seem
to be able to make it work. Is that what it's for? Is there a trick
to it?

From your description it sound like the stripper for flat cable. It will
remove the outer jacket and leave the individual conductors in place.
There may be a stop there too. Ideally, you want to strip about 1/4" off
the end of the flat cable to insert into the RJ type plug. When you
crimp the plug the insulation will be displaced by the condutors.
